About the Role:
As a Lead Care Manager Supervisor, you will work closely with the Director and fellow Lead Supervisor to oversee compliance with the ever-evolving NYS policies, ensuring that our program remains in alignment with regulatory standards. Your leadership will be pivotal in managing operational workflows, enhancing the quality of program oversight, and ensuring optimal support for our care managers. You will supervise a team of care managers handling high-acuity, complex cases, ensuring their professional growth and maintaining a high standard of care for our members. We are a dynamic, female only administrative team managing a growing Health Home Care Management program. While our care managers serve members across New York State, working remotely in the field, our supervisors and program administrators operate out of our office in Lakewood, NJ. Our office culture is collaborative and solutions-driven, where sharing ideas and improving processes is key. We are seeking an independent leader who thinks creatively and can develop innovative strategies for enhancing both the team’s effectiveness and the quality of care we deliver.Key Responsibilities:
- Collaborate closely with the Director and other Lead Supervisor to ensure program compliance with changing NYS policies and guidelines.
- Lead the operational management of workflows, ensuring high-quality oversight and streamlined processes across the team.
- Supervise a team of at least 8 Care Managers, ensuring they are well-equipped to manage complex, high-acuity cases, and that their performance aligns with program goals.
- Conduct regular one-on-one and group supervision sessions, providing support, guidance, and professional development opportunities for Care Managers.
- Manage the professional growth of Care Managers, identifying training needs, offering constructive feedback, and advocating for career development.
- Oversee and ensure timely and accurate documentation, including case reviews, compliance with quality assurance standards, and adherence to regulatory requirements.
- Foster a culture of teamwork, accountability, and positive communication among the care management team.
- Participate in strategic planning with the Director and Quality Assurance & Compliance teams, using data and feedback to drive program growth and enhance service delivery.
- Supervise the intake process, ensuring all new members are assigned to the appropriate Care Manager and have their eligibility confirmed prior to enrollment.
- Coordinate and ensure compliance with mandatory trainings, such as CANS-NY, and ensure all team members meet required qualifications.
- Support audit preparation and ensure all necessary documentation is in place for program reviews.
- Maintain a small caseload as necessary, managing and ensuring timely billing, service documentation, and program compliance.
Minimum Requirements:
- Master’s Degree in a related field (Human Services, Social Work, Public Health, etc.) preferred.
- At least 2-3 years of experience supervising or providing case management services, particularly in high-acuity settings.

- Strong leadership abilities with a proven track record of motivating teams and ensuring compliance with agency policies and regulatory standards.
- Decisive, with the ability to assess situations and develop effective solutions quickly.
- Exceptional organizational skills, with the ability to manage multiple priorities and meet deadlines.
- Excellent communication skills, both written and verbal, with the ability to handle sensitive information with tact and professionalism.
- High proficiency in the use of software and calendar systems to track team performance and ensure workflow efficiency.
- In-depth knowledge of federal, state, and local regulations related to care management and healthcare compliance.
- Commitment to maintaining confidentiality and ensuring adherence to HIPAA policies.
