Here at Anchor Health we looking for qualified candidates, to join our growing team of Case Managers / Retention Specialist

Responsibilities Includes:

• Serve as the primary point of contact for clients
• Coordinate and manage the comprehensive care plans for clients
• Develop and implement strategies to enhance client retention
• Maintain regular communication with clients, their families
• Keep detailed and accurate records of client interactions, and care plans
• Address and resolve any issues or challenges that may arise during the course of care
• Monitor and evaluate the quality of care provided, identifying areas for improvement and implementing necessary changes to enhance service delivery.
