Overview

Responsibilities Includes:

-Serve as the primary point of contact for clients and families
-Coordinate and manage the comprehensive care plans for clients
-Develop and implement strategies to enhance client retention
-Maintain regular communication with clients, their families
-Keep detailed and accurate records of client interactions, and care plans
-Address and resolve any issues or challenges that may arise during the course of care
-Monitor and evaluate the quality of care provided, identifying areas for improvement and implementing necessary changes to enhance service delivery.
